PRIVACY AND NON-DISCLOSURE POLICY FOR
SPECIAL OPERATIONS QUIET PROFESSIONALS FOUNDATION, INC.

RECITALS
Whereas, Special Operations Quiet Professionals Foundation, Inc. is a Florida Not for Profit Corporation D/B/A Special Operations Warrior Golf Foundation (hereinafter referred to as the “SOWGF”) formed for the purpose of operating as a tax-exempt charitable organization under Section 501(c)(3) of the Internal Revenue Code; and
Whereas, the SOWGF is committed to honoring the privacy and confidentiality of all SOWGF Donors and SOWGF Participating Warriors by adopting a Privacy and Non-Disclosure Policy (hereinafter referred to as the “Policy”), a copy of which will be attached to and become a part of the Bylaws of SOWGF.
Article I
SOWGF Warrior Participants
1.01: Purpose. The purpose of this SOWGF Privacy and Non-Disclosure Policy as it applies to SOWGF Warriors is to protect all SOWGF Warriors against unauthorized use of personal information obtained by SOWGF in connection with the Warrior’s participation in the retreat activities sponsored by SOWGF.
1.02 Use and Restrictions. SOWGF retains the right to use this personal information in its ordinary course of business subject to the following restrictions:
1.03: Waiver: In the event SOWGF wishes to use any protected information or materials set forth above, for any purpose not otherwise allowed in this Policy, SOWGF shall have each relevant participating Warrior execute a Waiver and Consent Agreement in the form attached hereto as Exhibit A.
Article II
DONORS AND MENTORS
2.01: Purpose. The purpose of this Privacy and Non-Disclosure Policy as it applies to SOWGF Donors and Mentors is to honor and protect the privacy and confidentiality of all Donors and Mentors.
2.02: Use and Restrictions. It is understood that the personal information collected by SOWGF from Donors and Mentors is used by SOWGF primarily to process the payment of contributions made by Donors and Mentors and provide them with a receipt and other substantiation of the charitable nature of their gift. However, SOWGF shall also use the e-mail addresses to answer emails received from Donors and Mentors and to send to donors and mentors periodic news emails and notices about SOWGF [Note: A Donor or Mentor is only sent the news email if they advise SOWGF that they wish to be included by using the sign-up box on our website]. Accordingly, SOWGF retains the right to use the personal information for the specific matters set forth above and more broadly in its ordinary course of business subject to the following restrictions:
2.03: Waiver. In the event SOWGF wishes to use any protected information or materials set forth above for any purpose not otherwise allowed in this Policy, SOWGF shall have each Donor or Mentor execute a Waiver and Consent Agreement in the form attached hereto as Exhibit A.
Article III
ADDITIONAL PROVISIONS
3.01: Scope: All board members, advisory board members, officers, employees, and volunteers shall be required to review this Policy and agree to abide by its terms and provisions.
3.02: Amendment: This policy may be amended by the Executive Committee. Upon approval the Secretary shall publish and distribute the changes to all parties set forth in Subparagraph 3.01 above.
3.03: Regulatory Requirements: Notwithstanding anything to the contrary contained herein, this Policy shall not restrict release or report of any information required by the Internal Revenue Service or any other national or state regulatory authority.
CERTIFICATE OF ADOPTION THE
PRIVACY AND NON-DISCLOSURE POLICY
The undersigned, being the Secretary of the Special Operations Quiet Professionals Foundation, Inc., D/B/A Special Operations Warrior Golf Foundation does hereby certify that the attached Privacy and Non-Disclosure Policy was approved by the Board of Directors on the 28th day of February, 2021 and constitutes a full and complete copy of said Policy
